For new patients, please click on the link below to download the New Patient Questionnaire so you can have it filled out and ready at your first visit. To view and print this material, you must have a copy of Adobe Acrobat installed on your computer. If you do not, please click on the Adobe Logo and you will be taken to their download site. This application is free.

If you have already been evaluated by another physician and have records related to the condition you are seeking treatment for, please have them sent to our office by completing the release of medical records form by selecting the link below. Send this form to your provider so they can fax the records to our office prior to your visit. We try to accomplish as much as possible during your initial visit. Please do not take any antihistamine containing medications for at least three days before your appointment. Antihistamines will interfere with allergy testing if that is required. Your first visit may require as much as two hours if testing is involved.

MINOR PATIENTS (< 18 years old) MUST BE ACCOMPANIED BY AN ADULT FOR PATIENT VISITS.

Requests to release medical records must be requested on the forms below. Please do not send clinical information by email as this is not private or secure.
